Questionnaire

If your child is demonstrating some of the signs described on the Signs page, the issues may be due to neuro-developmental delay or neuro-motor immaturity. In order to start to try to work out the reasons behind your child’s issues, your next step is to complete and submit the free Screening Questionnaire.

The purpose of this questionnaire is to identify if there is a neurological basis to your child’s problems. Your answers will indicate if further assessment is required.

Specific Learning Difficulties, problems with movement and behaviour, and emotional issues can be caused by many factors and can appear for a variety of reasons. It is useful to try and isolate these factors which may have led to your child’s problems.

The Screening Questionnaire looks at the antenatal history, the birth of your child and your child’s early years and schooling to the present day.
